Protected Health Information

It is your responsibility to ensure patient confidentiality. At no time should any Protected Health Information (PHI) as interpreted by the U.S. Health Insurance Portability and Accountability Act (HIPAA) be entered into ClinPrep. Real identifiers, such as those listed below, must never be entered into ClinPrep:

  1. Real Patient Names or initials
  2. Patient addresses, in whole or part
  3. Dates, such as birth date, discharge date, date of death, etc.
  4. Email addresses or any other internet identifier
  5. Phone Numbers
  6. Social security numbers
  7. Medical record or visit numbers
  8. Health insurance or account numbers
  9. Patient images or other biometric patient data
  10. Any other information that could be used to uniquely identify a patient
